Below are the steps to enroll with EDGE Teen Services.

Fill Out The Registration Form:
The digital student registration form for the 2025-26 academic year needs to be completed prior to the second visit to either Teen Center.
The EDGE@Work Community Service registration form for the 2025-26 academic year must be completed prior to the first outing.
Each parent/guardian must read the EDGE Code of Conduct and acknowledge it when signing the form.
For any parent/guardian who consents to allow their teen to receive counseling services, they must fill out this additional digital Consent for Treatment form.

Who Can Refer to a Teen?
Anyone who cares about a teen’s well-being: Parents, guardians, teachers, counselors, social workers, coaches, neighbors, and more.
If a teen could use connection, support, or a positive place to be—your referral matters.
